Enroll Your Child at JMA

Dear Parents,

Please also include the following documents when you return the completed enrollment forms:

• A copy of your child’s Social Security Card
• A copy of your child’s Birth Certificate
• A copy of your child’s most recent Report Card
• A copy of your child’s Shot Records

We must have these documents before your child enters Jean Massieu Academy.
